MAP Team
MAP is short for 'making a plan'. The team is made up of individuals from local community agencies that work with children and youth. The purpose of MAP is to review cases concerning children and youth (up to 21 years of age) who have a serious emotional disturbance/ serious mental illness and who are at risk for an inappropriate out-of-home placement due to the lack of access to or availability of needed services and supports in the home and community.
What is the goal of MAP TEAM?
To divert children and youth from an out of home placement.
What is the primary task of MAP TEAM?
To link families to available resources in the community that will help keep children and youth from inappropriately being placed out-of-home.
What happens when a referral is made?
Upon completion if referral forms, it is decided if the referral can be served by MAP Team. If they can, the case is presented at the next meeting for review by the team. It is at this time community resources are addressed and recommendations are made to divert out-of-home placement.
Is this confidential?
